Bridge by Folia Promises to Put Patients and Families One Step Closer to Precision Medicine

Folia Health, the home-reported outcomes company that enables patients and their families to use their observations to take control of their healthcare, has just announced Bridge by Folia, their new provider program. Bridge will help patients  families communicate their observations of how well treatments are working to their care teams, in an effort to improve the evidence-base available for clinic visits.
